Looking for the Right Support?
We welcome referrals and enquiries for children, young people and adults requiring disability support across Perth. We accept enquiries directly from participants and families as well as referrals from professionals.
Eligibility and funding depend on the participant’s individual NDIS plan and circumstances.
Fields marked with an asterisk (*) are required. The NDIS Number is optional – you can still refer if a participant’s plan is not yet approved.
Contact our team and we can discuss the participant’s support requirements before a formal referral is submitted.
